Caring for patients with life-limiting illnesses and those who are in their final days and hours of life can be some of the most difficult, yet most rewarding, nursing work. Patients, their families, and communities require care that honors all aspects of their physical and psycho-spiritual selves. Truly, holistic care can be challenging to provide, but when interprofessional teams can successfully assess and intervene in patient's lives, keeping people in a home care environment is often the most comfortable and preferred setting for care, which in turn increases the quality of life, as well as the experience of dying for both patients and their families.
