Increasing patients’ ability to manage their health independently is the primary goal of home care. The home care nurse faces significant challenges in building this autonomy because most patients do not have the literacy and math skills necessary to seek and understand their healthcare. Healthcare professionals, particularly home care clinicians, bear the responsibility of modifying their language and data presentation to meet each patient's individual level of understanding. This learning module will explore the disconnect between patient and healthcare professional and its impact on health outcomes. Techniques to improve and assess understanding learned here will improve your communication and your patient's health outcomes.
