This clinical documentation education assessment-based education has been designed as part of a physician-led, provider-focused initiative, called Optimizing Patient Acuity Capture (OPAC). The focus of this education is to ensure that you have a basic understanding of the key terminology and documentation best practices for full and accurate capture of the severity of illness and the health risks of your patients.

- Recognize how adhering to documentation rules/best practices results in fewer inquiries from hospital team staff, seeking clarity of patient status and care plans.
- Recognize how OPAC will positively impact you, your physician group, and specialty department.
- Recognize how the use of specific clinical terminology optimizes the full and accurate capture of patient acuity.
- Recognize that commonly used clinical terminology often fails to support the risk/severity adjustment used for Patient/Safety/Quality Reporting.
